Nurse Practitioner – Weekend On-Call

This is a fully remote position, open to applicants in United States.

📋 Description

• Evaluate the medical and mental health requirements of patients and develop proactive care plans.

• Identify and address obstacles to care while managing both acute and chronic patient conditions.

• Deliver personalized and coordinated healthcare services to residents in long-term care.

• Prescribe medications and arrange for laboratory tests, diagnostic procedures, and consultations.

• Observe patients’ adherence to treatment and their responses, adjusting care plans as necessary.

• Collaborate with RN case managers as part of a cohesive care team.

• Partner with facility collaborators and health plan leadership to execute TruHealth’s national care model.

• Consult with a supervising physician when deemed appropriate.

• Provide training and guidance to facility staff as needed.

⛳️ Requirements

• FNP, AGNP, AHACNP, or PA license is mandatory.

• Must possess a degree from an accredited APRN or PA program.

• A minimum of 3 years of experience in clinical nursing or rehabilitation within geriatric populations.

• Experience in Primary Care focused on Geriatric Medicine is preferred.

• Preferred experience in Long Term Care settings.

• Proficiency in Electronic Health Records is required.

• Familiarity with Microsoft applications, including Word, Outlook, and Excel.

• Capability to assess and treat both acute and chronic conditions.

• Ability to prescribe medications and manage lab tests, diagnostic procedures, and consultations.
