Nurse Practitioner – Weekend On-Call

This is a fully remote position, open to applicants in Louisiana.

📋 Description

• Deliver tailored and coordinated healthcare to long-term care residents enrolled in specialized Medicare Advantage plans.

• Evaluate patients’ medical and mental health requirements and create proactive care plans.

• Manage patients with both acute and chronic conditions using independent judgment, consulting a supervising physician when necessary.

• Ensure adherence to local, state, and federal regulations concerning clinical services.

• Prescribe medications and arrange for laboratory work, diagnostic procedures, and consultations.

• Oversee treatment adherence and patient reactions, adjusting care plans as required.

• Collaborate with RN case managers within an integrated care team.

• Partner with facility stakeholders and health plan leadership to execute the national model of care.

• Provide training and guidance to facility staff as needed.

⛳️ Requirements

• FNP, AGNP, AHACNP, or PA license is mandatory.

• Degree from an accredited APRN or PA program is required.

• A Master’s degree or higher is essential.

• Minimum of 3 years’ experience in clinical nursing or rehabilitation within geriatric populations.

• Preferred experience in primary care or internal medicine, ideally with a focus on geriatrics.

• Proficiency with Electronic Health Records.

• Familiarity with Microsoft applications, including Word, Outlook, and Excel.

• Experience in primary care related to geriatric medicine is preferred.

• Experience in long-term care is essential.

• APRN Advanced Practice RN certification/license is required.
