Nurse Practitioner – Weekend On-Call

This is a fully remote position, open to applicants in Iowa, +1 more state.

📋 Description

• Deliver personalized and coordinated healthcare services to members of Medicare Advantage plans residing in nursing homes or assisted living facilities.

• Evaluate the medical and mental health requirements of patients and devise proactive care plans.

• Utilize independent clinical judgment to manage both acute and chronic health conditions.

• Ensure adherence to clinical-service regulations at local, state, and federal levels.

• Prescribe medications and arrange for laboratory tests, diagnostic procedures, and consultations.

• Track treatment adherence and patient responses, adjusting care plans as necessary.

• Collaborate effectively with supervising physicians, RN case managers, facility partners, and health plan leadership.

• Execute TruHealth’s national care model.

• Provide guidance and training to facility staff as needed.

• Enhance the quality of life for residents and minimize unnecessary hospital admissions.

⛳️ Requirements

• A valid FNP, AGNP, AHACNP, or PA license is required.

• A degree from an accredited APRN or PA program is necessary.

• A minimum of 3 years of experience in clinical nursing or rehabilitation with geriatric populations.

• Proficiency with Electronic Health Records.

• Familiarity with Microsoft Office applications, including Word, Outlook, and Excel.

• Experience as an advanced practice provider, either as a nurse practitioner or physician assistant.

• Prior experience in primary care or internal medicine is preferred.

• A focus on geriatrics is preferred.

• A Master's degree or higher is required.

• APRN Advanced Practice RN certification/license is mandatory.
