Nurse Practitioner – Virtual Health Assessment

This is a fully remote position, open to applicants in Arizona, +16 more states.

📋 Description

• Conduct preventative care screenings, medication reconciliation, diagnosis, health coaching, referrals, and treatment when clinically warranted.

• Reach out to members with chronic illnesses based on historical and anticipated health information.

• Engage with members who may be less likely to visit their primary care physician.

• Identify ways to enhance members’ access to care with a licensed provider.

• Receive cross-training in urgent care and/or virtual primary care service lines.

• Interact with members via virtual platforms, including virtual assessments, diagnoses, and treatments.

• Adhere to all relevant laws and regulations.

• Execute additional tasks as assigned.

• Report directly to the Director of Virtual Health Assessment.

⛳️ Requirements

• Possession of a PA or DNP, FNP, ENP degree from an accredited institution.

• Board Certification (AANP, ANCC, or NCCPA) is required.

• Compact RN license (for Nurse Practitioners only).

• Experience in Clinical Documentation Improvement and/or management of HEDIS quality measures.

• Must hold a full license in at least two of the following states: FL, TX, OH, or NJ.

• Capability to obtain licensure in additional states as necessary.

• A minimum of 3 years of experience in outpatient Family Medicine, Internal Medicine, or Primary Care.
