Nurse Practitioner – Virtual Health Assessment

This is a fully remote position, open to applicants in Arizona, +16 more states.

📋 Description

• Conduct preventative care screenings, medication reconciliation, diagnosis, health coaching, referrals, and treatment when clinically appropriate.

• Reach out to members who are more chronically ill based on historical and presumed health information.

• Engage with members who are less likely to visit their primary care physician.

• Identify ways to enhance members' access to care with a licensed provider.

• Cross-train into urgent care and/or virtual primary care service lines.

• Interact with members through virtual platforms, including virtual assessment, diagnosis, and treatment.

• Adhere to all relevant laws and regulations.

• Fulfill other duties as assigned.

• Report to the Director of Virtual Health Assessment.

⛳️ Requirements

• PA or DNP, FNP, ENP from an accredited program.

• Board Certification (AANP, ANCC, or NCCPA).

• Compact RN (NP only).

• Experience with Clinical Documentation Improvement and/or managing HEDIS quality measures.

• Must hold a license in FL, AND either TX, OH, or NJ.

• Capability to obtain licensure in additional states.

• Over 3 years of experience in outpatient Family Medicine, Internal Medicine, or Primary Care.

• Must reside in Arizona, Arkansas, California, Colorado, District of Columbia, Florida, Georgia, Illinois, Massachusetts, Michigan, Nevada, New Hampshire, New Jersey, New York, North Carolina, Ohio, Pennsylvania, Texas, or Virginia.
