Nurse Practitioner – Virtual Health Assessment

This is a fully remote position, open to applicants in Arizona, +16 more states.

📋 Description

• Conduct preventative care screenings.

• Execute medication reconciliation.

• Identify and diagnose medical conditions.

• Offer health coaching, referrals, and treatment when clinically appropriate.

• Reach out to members who are more chronically ill based on historical and suspected health data.

• Engage members who may have a lower likelihood of visiting their PCP.

• Discover opportunities to enhance members' access to care with a licensed provider.

• Train in urgent care and/or virtual primary care service lines.

• Interact with members through virtual platforms, including assessment, diagnosis, and treatment.

• Adhere to all relevant laws and regulations.

• Carry out other assigned duties.

• Report directly to the Director of Virtual Health Assessment.

⛳️ Requirements

• PA or DNP, FNP, ENP from an accredited program.

• Board Certification (AANP, ANCC, or NCCPA).

• Compact RN (NP only).

• Experience in Clinical Documentation Improvement and/or managing HEDIS quality measures.

• Must hold a license in FL and either TX, OH, or NJ.

• Capability to obtain licensure in additional states.

• Over 3 years of experience in outpatient Family Medicine, Internal Medicine, or Primary Care.

• 8-hour shifts scheduled for 9 hours, including a one-hour lunch break.

• One late shift ending at 8 p.m. EST is required each week.

• Two Saturday shifts are required each quarter.
