
Nurse Practitioner PRN β Health Risk Assessments

This is a fully remote position, open to applicants in Kentucky.
β’ Conduct annual health risk assessments in residential environments or via telehealth.
β’ Execute thorough in-home health evaluations to identify both active and chronic health issues, as well as assess physical, mental, and social needs.
β’ Gather patient histories, perform physical examinations, and arrive at diagnoses.
β’ Generate written documentation of patient encounters in accordance with NCQA standards.
β’ Measure patient vital signs when necessary.
β’ Initiate case management referrals and liaise with primary care providers as needed.
β’ Communicate effectively with patients, caregivers, agency nurses, other healthcare providers, and vendors to ensure accurate diagnosis.
β’ Carry out clinical responsibilities while adhering to OSHA Universal Precautions.
β’ Uphold patient confidentiality at all times.
β’ Attend mandatory meetings and training sessions and engage in committees as requested.
β’ Participate in professional development initiatives and maintain relevant professional licenses and affiliations.
β’ Undertake additional job-related tasks as assigned.
β’ Active and unrestricted nurse practitioner license valid in coverage states.
β’ Board certification from one of the following: American Nurses Credentialing Center (ANCC), American Association of Nurse Practitioners (AANP), or National Commission on Certification of Physician Assistants (NCCPA).
β’ Current Basic Life Support (BLS) Certification.
β’ Current enrollment in Medicare and Medicaid programs.
β’ Valid and unencumbered driver's license with a clean driving record.
β’ Exceptional electronic health record (EHR) skills.
β’ Experience or training in geriatrics is preferred.
β’ Capability to manage varying weather conditions and navigate different home/residential environments.
β’ Full range of body motion, including the ability to lift and handle patients; standing, bending, walking, and climbing stairs are required.
β’ Ability to regularly lift or carry items weighing up to 50 pounds.
β’ Capacity to travel by automobile or van for up to 150 miles daily in both urban and rural areas.
β’ Willingness to drive, if necessary.
