Nurse Practitioner, Physician Assistant – In-Home Health

This is a fully remote position, open to applicants in Connecticut.

📋 Description

• Deliver in-home health assessments to members of health plans.

• Interact with members within their residences and foster trustworthy relationships.

• Conduct visits lasting approximately 45 minutes, which include reviewing medication and medical history along with a physical examination.

• Execute additional diagnostic tests as directed by the member’s health plan, such as diabetic eye examinations and spirometry.

• Perform virtual consultations as required.

• Offer Signify Health services, encompassing diagnostic and preventive procedures like spirometry, diabetic eye tests, and bone density assessments.

• Travel locally and within the states where licensed as necessary.

• Assist in identifying care gaps and connecting members with suitable healthcare resources.

• Become part of a national network of healthcare professionals dedicated to enhancing health outcomes.

⛳️ Requirements

• Hold an active, unrestricted license(s) in the relevant coverage area(s).

• Board certification is mandatory.

• Multi-state licenses are preferable.

• Willingness to travel locally and within licensed states as required.

• Capability to conduct visits in private homes that may have household pets, exposure to secondhand smoke, and airborne allergens or irritants.

• Proficient in using an iPad and clinical workflow for assessments.
