Nurse Practitioner, Physician Assistant

This is a fully remote position, open to applicants in Minnesota.

📋 Description

• Conduct health evaluations in the homes of members.

• Perform visits lasting approximately 45 minutes.

• Engage in one-on-one discussions with members to address their health inquiries.

• Review patients' medications and medical histories.

• Carry out physical assessments.

• Administer additional diagnostic tests as directed by the health plan, including diabetic eye examinations and spirometry.

• Utilize an iPad and clinical workflow for each assessment.

• Travel nationwide to connect individuals with the necessary care.

• Travel locally and within licensed states as required.

• Conduct virtual consultations when necessary.

• Provide Diagnostic & Preventive Services, such as spirometry, diabetic eye exams, and bone density screenings.

• Establish trusted relationships and contribute to enhanced health outcomes.

⛳️ Requirements

• Must be a Nurse Practitioner or Physician Assistant.

• Hold an active, unrestricted license in the relevant coverage areas.

• Board certification is mandatory.

• Multi-state licenses are preferred.

• Willingness to travel locally and within licensed states as necessary.

• Capability to conduct virtual visits when required.

• Must be able to safely perform duties in homes that may have household pets, secondhand smoke, and other airborne allergens or irritants.
