Nurse Practitioner, Physician Assistant

This is a fully remote position, open to applicants in Maine.

📋 Description

• Deliver in-home health assessments to members of health plans.

• Conduct visits lasting around 45 minutes, which include reviews of medication, medical history, and physical evaluations.

• Execute additional diagnostic tests as prescribed by the health plan, such as diabetic eye examinations and spirometry.

• Utilize an iPad and clinical workflow for each assessment.

• Establish trusted relationships to enhance health outcomes.

• Travel across the nation to connect individuals with appropriate care.

• Provide services from Signify Health, including Diagnostic & Preventive Services products.

• Carry out virtual visits when necessary.

• Travel locally and within the states you are licensed to practice as required.

⛳️ Requirements

• Possession of an active, unrestricted license(s) in the relevant coverage areas.

• Board certification is mandatory.

• Multi-state licenses are preferred.

• Capability to cover Aroostook and its surrounding areas.

• Willingness to travel locally and within licensed states as necessary.

• Ability to conduct virtual visits as required.

• Competence to safely perform duties in homes that may have pets, secondhand smoke, and airborne allergens or irritants.
