Nurse Practitioner – Part-Time

This is a fully remote position, open to applicants in Florida.

📋 Description

• Deliver comprehensive remote virtual care management for serious illnesses to patients and their families in home-based, facility-based, and inpatient hospice, palliative care, PACE, and other programs.

• Collaborate with medical personnel and interdisciplinary team members to ensure the best possible patient care.

• Conduct virtual assessments to evaluate the needs of patients, families, and caregivers.

• Identify, manage, prescribe medications and equipment as necessary.

• Modify and support care plans in accordance with organizational policies and nursing practice regulations.

• Evaluate caregiver capabilities and provide necessary education and assistance.

• Respond to clinical inquiries and assist staff with orders, prescriptions, and patient requirements.

• Maintain access to electronic medical records (EMRs), electronic prescribing systems, and other essential applications.

• Promptly document interactions within the relevant EMR.

• Communicate and collaborate with primary teams to ensure seamless continuity of care.

• Assist in discharge planning, including managing medications, orders, and instructions.

• Engage in QAPI activities, medical record evaluations, and peer reviews.

• Stay informed about current research and trends in hospice and palliative medicine.

• Pursue continuing education and secure and maintain hospital/facility credentials.

• Take part in departmental and medical staff meetings.

• Interact with team members, patients, families, caregivers, providers, and staff while adhering to HIPAA regulations.

• Collaborate with interdisciplinary teams to create and execute personalized care plans.

• Provide education to staff and patient/family education.

• Deliver outstanding customer service and support organizational changes.

• Be available and provide coverage as required.

• Meet organizational productivity benchmarks and implement telemedicine billing protocols when applicable.

⛳️ Requirements

• At least 3 years of recent clinical experience, with significant exposure to hospice, palliative care, and/or acute inpatient care.

• A Master’s or Doctorate in Nursing from an accredited Family or Adult Nurse Practitioner Program.

• National certification, licensure, and prescriptive authority for advanced nursing practice in the home state, as well as any additional states where care is provided.

• Licensure and prescriptive authority must be acquired and sustained for all necessary jurisdictions.

• Applications for required licenses should be submitted within 60 days of hire or notification.

• Basic Life Support certification from a recognized organization by the date of hire.

• Compact RN license in the home state by the date of hire.

• Availability for various shifts, including nights, evenings, weekends, and/or holidays.

• Proficient in using electronic medical records, electronic prescribing systems, patient health information systems, computer telephony systems, and necessary applications.

• Competent in Microsoft Word, Excel, and PowerPoint.

• Capability to maintain reliable, HIPAA-compliant high-speed (100mb) internet with a hardwired backup.

• Ability to sustain a HIPAA-compliant, temperature-controlled, and noise-free work environment.

• Advanced clinical judgment and problem-solving skills.

• Capacity to work independently as well as part of a team, manage multiple tasks, and perform under pressure.

• Comfort in working with terminally ill patients and emotionally charged patients/families.

• Agreement to post-offer and ongoing drug and/or alcohol testing.

• Continuous eligibility through the Florida Care Provider Background Screening Clearinghouse.
