Nurse Practitioner – In-Home Health

This is a fully remote position, open to applicants in Michigan.

📋 Description

• Conduct in-home health assessments for health plan members.

• Foster engagement with members and establish trustworthy relationships.

• Evaluate medications and review medical history.

• Execute physical assessments.

• Administer additional diagnostic tests as prescribed by the health plan, including diabetic eye examinations, spirometry, and bone density screenings.

• Facilitate virtual visits as required.

• Deliver Signify Health services, encompassing Diagnostic & Preventive Services.

• Travel locally and across licensed states as necessary.

• Assist in bridging care gaps and connecting members with appropriate healthcare resources.

⛳️ Requirements

• Possession of an active, unrestricted license(s) in the relevant coverage area(s).

• Board certification is mandatory.

• Multi-state licenses are preferred.

• Willingness to travel locally and within licensed states as required.

• Capability to conduct visits in private residences that may have household pets, secondhand smoke, and airborne allergens or irritants.

• Proficiency in using an iPad and navigating an intuitive clinical workflow.

• Qualification as a Nurse Practitioner or Physician Assistant.
