Nurse Practitioner, IL License Required

This is a fully remote position, open to applicants in Illinois.

📋 Description

• Address the care needs associated with the Transitions of Care program.

• Engage in telephonic outreach and arrange video or phone consultations for patients who have recently been hospitalized or discharged.

• Execute medication reconciliation and conduct symptom assessments.

• Request necessary follow-up laboratory tests, medications, or supplies.

• Establish rapport, clarify Galileo's care model, and link patients to appropriate services.

• Facilitate care coordination among hospitals, emergency departments, pharmacies, and community providers.

• Work collaboratively with the nursing team.

• Achieve personal performance goals while contributing to team-oriented population health objectives.

• Deliver care during 4-hour shifts, available across mornings, afternoons, evenings, and overnight on both weekdays and weekends.

⛳️ Requirements

• Must be a Certified Nurse Practitioner specializing in Adult, Family, or Geriatrics.

• Hold an active state license in Illinois, with the capability to secure additional state licenses.

• Possess over 5 years of post-graduate clinical experience in Primary Care or Geriatrics.

• Up to 2 years of experience may be substituted with work in a Skilled-Nursing facility or Long Term Care facility.

• Direct experience with Medicare and Medicaid populations managing complex, comorbid conditions is essential.

• Strong skills in motivational interviewing.

• Comfortable in a digital-first setting, with experience in or a strong interest in virtual care delivery.

• Proficient in technology.

• Exceptional written and verbal communication abilities.

• Bilingual in English and Spanish, including cultural competence, is highly desirable.

• Familiarity with Google Suite and Slack is advantageous.

• Additional licenses in Texas, Pennsylvania, New Jersey, New York, Massachusetts, Minnesota, Louisiana, and/or South Carolina are a plus.
