This is a fully remote position, open to applicants in Idaho, +3 more states.
β’ Conduct thorough evaluations to establish, confirm, and stage dementia diagnoses.
β’ Develop and implement evidence-based care plans that address clinical, behavioral, safety, and caregiver-support objectives.
β’ Keep up-to-date with advancements in dementia care and share knowledge across the team.
β’ Identify opportunities for enhancing the care model and technical platform.
β’ Manage a panel of dementia patients and their caregivers over time.
β’ Assess the unique needs of patient-caregiver pairs and tailor interventions to their priorities.
β’ Monitor cognitive changes and environmental stressors to avert acute escalations, unnecessary emergency department visits, and hospitalizations.
β’ Collaborate with the interdisciplinary team, which includes Neurologists, Geriatricians, Care Navigators, Registered Nurses, Nurse Practitioners, Licensed Clinical Social Workers, and Pharmacists.
β’ Partner with teams to meet clinical quality, utilization, and patient/provider satisfaction benchmarks.
β’ Engage in workflows that ensure reliable 24/7 access to care.
β’ Report to the Medical Director and assist in the hiring and training of new providers as the organization grows.
β’ Work across functions to refine processes and tools for individuals living with dementia and their caregivers.
β’ Board-certified Psychiatric Mental Health Nurse Practitioner (PMHNP-BC) or Adult-Gerontological Nurse Practitioner (AGPCNP-BC).
β’ Graduate from an accredited US Nursing School and an accredited Nurse Practitioner Program.
β’ Established clinical experience in geriatrics, palliative care, or memory care.
β’ Extensive knowledge in dementia and related cognitive disorders.
β’ Proven experience in advance care planning and serious illness communication with patients and caregivers.
β’ Experience collaborating with interdisciplinary care teams.
β’ Previous experience in a fast-growing startup or digital health setting.
β’ Exceptional communication skills in a remote-first environment.
β’ Familiarity with asynchronous tools such as Slack and Notion.
β’ Comfortable providing care via telehealth.
β’ Strong problem-solving abilities and a flexible, innovative mindset.
β’ Active, unrestricted license in OR, WA, ID, and OH.
β’ Strong leadership and communication capabilities.
β’ Enthusiasm for building, launching, and refining a novel care model and technical platform.
