Nurse Practitioner, CT License Required

This is a fully remote position, open to applicants in United States.

📋 Description

• Address care requirements for the Transitions of Care program

• Engage in telephonic outreach and conduct scheduled video/phone consultations for patients who have recently been hospitalized or discharged

• Execute medication reconciliation and conduct symptom assessments

• Arrange necessary follow-up labs, medications, or supplies

• Establish trust, clarify Galileo's care model, and link patients to appropriate services

• Coordinate care among hospitals, emergency departments, pharmacies, and community service providers

• Work collaboratively with the nursing team

• Achieve individual performance objectives and contribute to team-level population health initiatives

• Commit to 4-hour shifts during mornings, afternoons, evenings, or overnight on weekdays and weekends

⛳️ Requirements

• Certified Nurse Practitioner in Adult, Family, or Geriatrics

• Active state licensure in CT, with the capacity to acquire additional state licenses

• Over 5 years of post-graduate clinical experience in Primary Care or Geriatrics

• Up to 2 of those years may be offset by experience in a Skilled-Nursing or Long Term Care facility

• Direct experience interacting with Medicare and Medicaid populations managing complex, comorbid conditions

• Proficient in motivational interviewing techniques

• Comfortable in a digital-first setting, with experience in or a strong interest in virtual care delivery

• Strong technological proficiency; familiarity with Google Suite and Slack is advantageous

• Excellent written and verbal communication abilities

• Bilingual in English and Spanish, with cultural fluency, is a significant plus

• Available to work a minimum of 24 hours per week

• Providers are expected to work weekends on a monthly basis
