Nurse Practitioner

This is a fully remote position, open to applicants in Virginia.

📋 Description

• Deliver primary and acute care using telephone and tele-video methods.

• Create and execute clinical care plans for adult patients with chronic and complex health issues.

• Collect patient histories, conduct physical examinations, and perform necessary diagnostics.

• Formulate and implement treatment plans tailored to patient goals and current health conditions.

• Recognize and address gaps in patient care.

• Attend to the physical and psychosocial needs of patients and their families with support from an interdisciplinary team.

• Provide education to patients and families regarding medications, potential side effects, illness progression, dietary needs, nutrition, adherence, and crisis prevention strategies.

• Facilitate care coordination among company physicians, other medical professionals, patients, families, and clinical team members.

• Keep patient medical records and documentation compliant with regulations and company policies.

• Engage in continuing education opportunities.

• Prescribe medications as allowed by state regulations.

• Collaborate to create intricate care plans that align with patient health status, objectives, and values.

• Offer both clinical and non-clinical support to patients receiving home and community-based services.

⛳️ Requirements

• Master's degree in Nursing.

• Current, active national Nurse Practitioner (NP) certification.

• A valid, current, and unrestricted Family or Adult Nurse Practitioner license in Virginia.

• Active RN Compact license.

• Minimum of 2 years of experience managing complex care cases.

• Proficiency in working with Electronic Medical Records (EMR).

• Candidates should be experienced Nurse Practitioners.

• DEA registration or eligibility is preferred.

• Active Medicaid number in Virginia is highly preferred.

• Strong preference for experience managing complex care cases for patients with developmental disabilities and chronic illnesses.

• Must live within a reasonable commuting distance from the job location(s), unless accommodations are made.

• Availability to work Monday through Friday, from 8:00 am to 5:00 pm EST or CST, with a rotating on-call schedule.
