Nurse Practitioner

This is a fully remote position, open to applicants in Kentucky, +1 more state.

📋 Description

• Collaborate with the company's physicians, other healthcare providers, patients, and family members to create comprehensive care plans.

• Deliver both clinical and non-clinical support to patients.

• Offer primary and urgent healthcare through phone and tele-video channels.

• Design and execute clinical care plans for adult patients dealing with chronic and complex health conditions.

• Collect patient histories, conduct physical examinations, and perform necessary diagnostics.

• Formulate and implement treatment plans aligned with patient objectives and current health status.

• Identify and address gaps in patient care.

• Meet the physical and psychosocial needs of patients and their families in collaboration with an interdisciplinary team.

• Educate patients and their families regarding medications, potential side effects, illness progression, nutrition, adherence, and crisis prevention strategies.

• Coordinate care and resources with clinical team members, physicians, and medical providers.

• Maintain accurate patient medical records and documentation.

• Engage in ongoing education and training.

• Prescribe medication as authorized by state regulations.

• Work Monday to Friday, from 8:00 a.m. to 5:00 p.m., with rotating on-call responsibilities 2 to 3 days per month from 5:00 p.m. to 8:00 a.m.

⛳️ Requirements

• Master's degree in Nursing.

• Current national Nurse Practitioner (NP) certification.

• Valid, active, and unrestricted Family or Adult Nurse Practitioner license in Indiana.

• Valid, current, and active Registered Nurse (RN) license.

• Proficiency with Electronic Medical Records (EMR).

• A minimum of 2 years of experience in managing complex care cases.

• Preferred: Active RN Compact license.

• Strongly preferred: Experience managing complex care cases for patients with developmental disabilities and chronic illnesses.

• Preferred: DEA registration or eligibility.

• Highly preferred: Bilingual in Spanish.

• Highly preferred: Active Medicaid number in Indiana.

• Must live within a reasonable commuting distance from the job location(s), unless otherwise accommodated as required by law.

• Required to complete designated in-person training sessions.

• COVID-19 and Influenza vaccinations are mandatory for specific patient-facing roles, subject to acceptable exemptions.
