Nurse Navigator – Urology Specialty Clinic

This is a fully remote position, open to applicants in Michigan.

📋 Description

• Serve as a connection point between patients, healthcare professionals, and physicians.

• Deliver care management and coordination for adult patients dealing with intricate conditions, social issues, and educational requirements.

• Work collaboratively with primary care providers, specialists, patients, and their families.

• Evaluate, plan, implement, coordinate, monitor, and assess available options and services.

• Enhance patients' health outcomes and reduce the frequency of hospital and emergency room visits.

• Promote quality improvement efforts and ensure compliance with safety standards.

⛳️ Requirements

• Active RN license in the State of Michigan.

• Preferred experience in managing complex chronic diseases.

• Bachelor of Science in Nursing is preferred.

• Ideally, five years of experience as a registered nurse.

• Must have vaccination or laboratory-confirmed immunity for Measles, Mumps, Rubella, and Varicella.

• Required to obtain a flu vaccine during the flu season of the year of hire and annually thereafter, or possess an approved medical or religious exemption.
