This is a fully remote position, open to applicants in United States.
• Coordinate and supervise the delivery of patient care for designated inpatient and outpatient groups.
• Evaluate patient needs and suggest care based on the severity of illness, required care, and suitable service levels.
• Work collaboratively with bedside nurses, providers, and interdisciplinary teams to create personalized care plans.
• Perform patient rounds and act as a resource for both patients and their families.
• Provide education prior to surgery, facilitate discharge planning, and conduct follow-up care post-discharge.
• Ensure that patients and their families comprehend treatment plans and discharge instructions.
• Assist in transitions of care and arrange post-acute services.
• Mentor and support nursing staff and other healthcare professionals.
• Foster professional accountability, adherence to regulations, patient safety, and continuous improvement.
• Delegate tasks and ensure accountability in care delivery.
• Advocate for quality improvement, outcome enhancement, safety, and operational performance initiatives.
• Aim to reduce the length of stay, maximize resource utilization, and enhance care efficiency.
• Analyze outcomes, identify opportunities for improvement, and implement corrective action plans.
• Monitor and report on performance indicators and clinical metrics.
• Track patient progression and document utilization management activities in electronic medical record systems.
• Leverage technology and data analytics for quality enhancement, risk management, and performance evaluation.
• Stay informed about insurance authorization, precertification, and reimbursement requirements.
• Support program expansion, development, and community outreach efforts.
• Establish relationships with physicians, healthcare partners, referral sources, community organizations, and stakeholders.
• Monitor referral patterns, market trends, and patient outcomes.
• Contribute to the structure, content, professional growth, learning, and collaboration of the program.
• Graduate of an accredited School of Nursing.
• Current Registered Nurse (RN) license in the relevant state of practice.
• A minimum of 2-3 years of nursing experience.
• Basic Life Support (BLS) certification.
• Bachelor of Science in Nursing (BSN) is preferred.
• Advanced Cardiac Life Support (ACLS) certification is preferred.
• Strong clinical assessment and critical thinking abilities.
• Excellent communication and interpersonal skills.
• Experience in care coordination and case management.
• Knowledge of utilization management and discharge planning practices.
• Capacity to lead multidisciplinary teams and influence outcomes.
• Strong organizational and time management abilities.
• Proficiency with electronic medical record systems, including Epic.
• Commitment to patient-centered care and service excellence.
• Physical ability to lift, push, and pull up to 50 pounds regularly.
• Capability to stand and walk for extended durations throughout the workday.
• Ability to bend, kneel, squat, reach, twist, and perform other physical movements essential for providing patient care.
