This is a fully remote position, open to applicants in South Carolina.
• Enable families of members to promote optimal wellness.
• Advocate for essential medical services throughout the healthcare continuum.
• Operate in a telephonic managed care environment for high-risk member demographics.
• Evaluate members’ physical, functional, psychological, environmental, educational, and financial requirements.
• Conduct needs assessments and create personalized care plans.
• Assign risk stratification based on medical and social complexities.
• Establish ongoing call frequencies to evaluate care plans.
• Oversee care plans, service delivery, and benefit sufficiency.
• Collaborate with providers on care coordination, including appointments, transportation, and durable medical equipment (DME).
• Educate members on their conditions, treatment plans, benefits, services, and how to access care.
• Track progress towards objectives and reassess changes in health status.
• Provide referrals to community resources and facilitate access and communication among services.
• Engage in interdisciplinary and client rounds to identify, clarify, or avert risks, quality concerns, and care plan discrepancies.
• Residency in South Carolina is mandatory.
• Must be a Registered Nurse (RN) with a current, unrestricted license in South Carolina.
• A minimum of three (3) years of clinical experience in a Level III or higher NICU is required.
• CCM certification must be obtained within the timeframe outlined by company policy.
• Experience with data entry into a database or electronic health record (EHR) is essential.
• Proficiency in computer use is required.
• Strong communication skills and organizational abilities are necessary.
• Self-motivation and a willingness to learn various tasks are essential.
• Must be well-organized and capable of prioritizing tasks effectively.
• Accuracy in spelling and documentation is a must.
• Proven ability to work independently and collaboratively within a team is required.
• Excellent interpersonal skills and telephone etiquette are necessary.
• Must maintain positive communication at all times.
• A commitment to providing exceptional customer service is essential.
• Bilingual proficiency in Spanish is strongly preferred.
• A college degree is preferred.
• Prior experience in a Case Management role is preferred.
