Nurse Case Manager

This is a fully remote position, open to applicants in Canada.

📋 Description

• Oversee and implement services related to Sentrex Health Solutions' patient support initiatives.

• Manage all facets of program assistance and serve as the primary contact for designated health care providers.

• Guarantee that patients can access support programs and prescribed therapies.

• Engage with patients, insurance providers, pharmacies, physicians, and other health care professionals to secure medication coverage and handle case management.

• Execute registration and enrollment activities for healthcare providers and patients.

• Conduct data entry related to the program.

• Address general inquiries through telephone and online platforms.

• Offer medical support, education on diseases and therapies, and guidance on disease management.

• Participate in specialized training sessions offered by the pharmaceutical team.

• Provide inbound and outbound telephonic support, maintenance calls, and adherence assistance.

• Implement case management protocols, which include clinical monitoring, data collection, follow-ups, compliance, therapy disruption monitoring, adverse event reporting, and coordination with primary health care providers.

• Collaborate with reimbursement specialists and physicians to secure therapy funding and explore alternative funding options.

• Attend patient meetings and conferences as needed.

• Identify and report adverse events to Health Canada, Med Info, and relevant third-party systems.

• Coordinate services between physicians’ offices, pharmacies, infusion service providers, and other stakeholders to resolve patient and physician-related issues.

• Engage in continuing education activities.

• Develop training materials and programs for new members of the Patient Support Program team.

• Facilitate coverage renewals and provide reimbursement support throughout the patient journey.

• Review, analyze, and validate patient information for accuracy and completeness.

• Schedule appropriate subsequent steps to ensure seamless service delivery.

• Follow up and advocate until coverage decisions are finalized.

• Perform additional responsibilities as designated by the Manager.

⛳️ Requirements

• Must hold a valid Registered Nurse designation in good standing with the College of Nurses in their respective province.

• 2-4 years of clinical experience is required.

• Bilingual communication skills in English and French are preferred.

• Experience in a Patient Support Program is a significant advantage.

• Background in immunology is considered an asset.
