This is a fully remote position, open to applicants in Canada.

📋 Description

• Oversee and manage intricate care coordination cases and activities to facilitate timely patient access, treatment advancement, and continuity of care.

• Perform assessments, assist in care planning, and enable transitions of care while addressing clinical, social, and access challenges.

• Collaborate with providers, pharmacies, payers, and internal stakeholders to eliminate access obstacles, synchronize subsequent steps, and ensure case continuity.

• Evaluate case trends, recurring problems, and workflow patterns to pinpoint root causes and propose process enhancements.

• Create and enhance job aids, care coordination templates, and documentation standards.

• Mentor less experienced team members in handling complex cases, escalation procedures, and documentation requirements.

⛳️ Requirements

• A Bachelor's degree in nursing, social work, health sciences, public health, healthcare administration, or a related discipline, or equivalent experience is required.

• A minimum of 4 years of experience in care management, case management, clinical support, patient services, or a related field is necessary.

• Certification in case management, care coordination, clinical specialty, or equivalent is preferred.
