Nurse Case Management Lead Analyst

📋 Description

• Oversee case management projects that improve patient outcomes and optimize care delivery.

• Create and execute strategies aimed at decreasing hospital readmissions and enhancing quality metrics.

• Work collaboratively with cross-functional teams to develop coordinated care plans.

• Evaluate data trends to pinpoint opportunities for process enhancement and cost efficiency.

• Guide and support nursing staff to promote professional development and excellence in patient care.

⛳️ Requirements

• Valid RN license in good standing, issued in the state of residence.

• At least 2 years of clinical nursing experience, which must include a minimum of 2 years in case management, care coordination, or prior authorization.

• Strong analytical abilities with the capacity to interpret healthcare data and utilize insights effectively.

• Exceptional communication and leadership skills.

• Bachelor's degree in Nursing is preferred.

• Preferred experience with value-based care models and population health strategies.

• Proficiency in case management software and electronic health records (EHR) is preferred.

• Access to home internet via cable broadband or fiber optic service with a minimum speed of 10Mbps download and 5Mbps upload.
