This is a fully remote position, open to applicants in New York, +1 more state.
• Oversee patients' specialized requirements based on their health conditions and/or recent healthcare usage.
• Take responsibility for assigned patients suffering from complex chronic illnesses, serious health issues, advanced frailty, or recent healthcare interactions.
• Assess patient needs through standardized evaluations and workflows.
• Create thorough nursing care plans collaboratively with patients and healthcare providers.
• Facilitate care coordination with Honest clinicians, network providers, contracted vendors, and community services.
• Establish compassionate relationships with patients and their families.
• Work alongside physicians, advanced practice providers, specialists, and multidisciplinary teams on case management strategies.
• Manage access to healthcare services across various providers and disciplines.
• Track care and identify cost-effective measures and alternative levels of care.
• Oversee patient admissions and discharges from hospitals and skilled nursing facilities.
• Foster relationships with hospitals, skilled nursing facilities (SNFs), home health agencies, and durable medical equipment (DME) vendors.
• Review and adjust nursing care plans based on quality and cost factors.
• Ensure continuity of care through effective transfers, discharge planning, and authorizations.
• Advocate for and provide education to patients and families.
• Design and implement personalized disease-state education plans.
• Address social determinants of health, including transportation, housing, and food resources.
• Engage in the collection and analysis of clinical outcomes and customer satisfaction data.
• Maintain precise patient care documentation.
• Participate in huddles, interdisciplinary team meetings, and patient case discussions.
• Represent Honest Health to patients, families, and the broader community.
• Carry out other related duties as assigned.
• Must hold a license in the state(s) where the position is located: New York and/or Michigan.
• Must have reliable access to high-speed internet.
• An active Registered Nurse license is required in Michigan and/or New York.
• Willingness and ability to obtain additional state nursing licenses as business needs arise.
• Experience in engaging patients virtually through phone, text, video, and other technologies is mandatory.
• Prior experience with electronic health records or health registries is required.
• Proficiency in Microsoft Office, including Excel, Word, PowerPoint, and Outlook is essential.
• A Bachelor’s of Science in Nursing is preferred.
• Certification as a Case Manager (CCM) is preferred.
• A minimum of 3 years of clinical practice experience is highly preferred.
• Previous case management experience with a senior population is preferred.
• Experience in disease management and/or working in a physician's office is highly preferred.
• Familiarity with PowerPoint is preferred.
