Nurse Care Manager

This is a fully remote position, open to applicants in United States.

📋 Description

• Oversee a designated patient panel, catering to each patient’s unique needs based on their health conditions, healthcare requirements, objectives, and preferences.

• Utilize nursing expertise and value-based care operations for comprehensive management of the patient panel.

• Build trusted and compassionate relationships with patients and their families, offering both clinical and emotional support.

• Act as an advocate and community liaison for patients as they navigate the healthcare landscape.

• Conduct assessments to identify the needs of patients and caregivers, including social determinants of health, and develop patient-centered care plans.

• Coordinate interdisciplinary care to ensure continuity, minimize fragmentation, manage the progression of kidney disease, and enhance provider collaboration.

• Monitor and review care management plans, adjusting interventions as needed.

• Adhere to evidence-based guidelines, workflow protocols, quality metrics, regulatory mandates, and payer stipulations.

• Collaborate with physician partners, community providers, APPs, and various clinical disciplines to create integrated care plans.

• Identify cost-effective strategies that support value-based care objectives and resource utilization.

• Facilitate education for patients and caregivers regarding treatment options.

• Support transitions of care and proactively resolve barriers.

• Participate in clinical huddles and patient care conferences.

• Engage in organizational process improvements and enhance care management workflows and patient engagement processes.

• Prepare reports and deliverables for stakeholders.

• Gather and present reports to program leaders regarding program effectiveness, patient outcomes, and patient/caregiver satisfaction.

• Execute other duties consistent with the role as assigned.

⛳️ Requirements

• Associate degree in nursing.

• A current RN License is mandatory; a Compact License is preferred.

• Experience in care management is required.

• Certification as a Case Manager is preferred.

• Proficient with the MS Office Suite, including Outlook and Teams.

• Ability to function effectively in a predominantly remote work environment.

• Home internet must provide a minimum download speed of 25 Mbps and an upload speed of 10 Mbps.

• Recommended connections include Cable, Fiber, or DSL, hardwired to the internet device.

• Employees must operate from a dedicated space that is free from ongoing interruptions to protect member PHI / HIPAA information.

• Availability to work from 8:30 a.m. to 5:00 p.m. ET or CT.

• Provide support for patients within the Central and Eastern Time Zones.
