Neurology Care Nurse Practitioner

This is a fully remote position, open to applicants in New Hampshire.

📋 Description

• Deliver direct clinical services to patients experiencing neurological disorders, memory issues, and/or dementia through telehealth platforms.

• Perform patient evaluations, formulate care strategies, and oversee continuous treatment in partnership with supervising physicians.

• Collaborate with neurologists and the wider healthcare team to guarantee seamless continuity of care.

• Provide education to patients and their families regarding diagnoses, treatment methods, and the progression of diseases.

• Ensure precise and timely clinical documentation in the EMR that complies with regulatory standards.

• Administer care to Local Infusion patients and work alongside physicians and the larger care team to enhance patient accessibility and minimize treatment delays.

⛳️ Requirements

• Residency in New Hampshire with the ability to work remotely from the state.

• Possession of an active, unrestricted Nurse Practitioner license in New Hampshire.

• Geriatric, adult-gerontology, or family practice certification is preferred.

• A minimum of 2 years of clinical experience in neurology, memory care, and/or dementia care environments.

• Strong clinical judgment and proficient clinical documentation abilities.

• Experience in managing a full patient panel independently while collaborating with physicians.

• Outstanding communication skills for interacting with patients experiencing cognitive decline and their caregivers.
