This is a fully remote position, open to applicants in Ohio.

📋 Description

• Build strong, professional relationships with members while conducting health risk assessments.

• Take part in the Integrated Care Coordination Team as necessary.

• Organize logistics to support the goals and interventions outlined in care plans to address immediate member needs.

• Confirm member eligibility, review previous enrollment history, demographics, and current health status prior to outreach.

• Carry out assessments by collecting information from members, families, providers, and other relevant stakeholders.

• Aid in the development and implementation of care plans and communicate assessment results to the Care Coordinator as required.

• Support health education initiatives, wellness materials, and coaching efforts.

• Ensure proper documentation aligns with Care Management program protocols and guidelines.

• Approach interventions with a focus on understanding each member’s perspective, unique needs, and how CareSource can assist.

• Empower members by ensuring they feel informed and supported by CareSource.

• Enhance processes to simplify members’ experiences and expedite assessment and follow-up timelines.

• Perform additional job responsibilities as assigned.


⛳️ Requirements

• A High School Diploma or General Education Diploma (GED) is mandatory.

• Preferably two (2) years of experience in either volunteer or paid roles in community settings, particularly with at-risk populations, providing service coordination.

• Proficient in Microsoft Office Suite, including Outlook, Word, and Excel.

• Sensitivity to and experience with diverse cultures.

• Strong interpersonal skills.

• Capability to work both independently and collaboratively within a team.

• Skill in identifying problems and opportunities and effectively communicating them to management.

• Developing knowledge of healthcare laws and regulations at local, state, and federal levels, as well as company policies related to case management practices.

• Exhibit compassion, support, and collaboration with members and their families.

• Self-driven and curious.

• Comfortable asking relevant questions.

• Ability to thrive in a fast-paced environment.

• Maintain and promote ethical conduct.

• Build positive relationships with all stakeholders.

• Awareness of community and state support resources.

• Organized, detail-oriented, and skilled in conflict resolution.

• Maintain composure and professionalism during emotionally charged situations.

• Uphold confidentiality and act in the best interests of the company.

• Demonstrated history of empathy and compassion towards individuals.

• Proven record of improving processes to facilitate easier experiences for those served.

• No licensure or certification is required.
