This is a fully remote position, open to applicants in North Carolina.
β’ Engage in consistent telephonic outreach to connect with identified and referred members for care management services.
β’ Manage and receive referrals from data reports, clinical care management team members, primary care providers (PCPs), and other service providers.
β’ Complete and document member screening instruments.
β’ Refer clinical requirements to the suitable clinical care management team member for evaluation and follow-up.
β’ Arrange face-to-face or telephonic follow-up encounters for members in care management.
β’ Achieve daily and monthly productivity and engagement benchmarks for the program.
β’ Fulfill assignments within designated timeframes.
β’ Inform the supervisor of any issues related to referrals, assignments, or duties.
β’ Execute tasks that ensure operations remain organized and efficient.
β’ Participate in departmental and corporate meetings.
β’ Adhere to CCNC goals, objectives, standards, departmental guidelines, company policies, and HIPAA regulations.
β’ Collaborate with members, providers, and staff to implement program interventions, document activities, and refer the appropriate members to professional staff.
β’ High School diploma or GED.
β’ A minimum of two years of prior work experience, preferably in a healthcare environment.
β’ Previous experience in a telephonic call center is preferred.
β’ Healthcare experience and familiarity with medical terminology are highly desirable.
β’ Bilingual candidates are preferred.
β’ Strong verbal and written communication skills.

β’ Critical thinking capabilities.
β’ Proficiency in motivational interviewing techniques.
β’ Understanding of care management principles.

β’ Ability to provide information in a culturally and linguistically appropriate manner.
β’ Familiarity with member or clinical data systems.
β’ Required computer skills, including proficiency in various office software and internet usage.
β’ Experience with MS Office software is preferred.
β’ Must comply with HIPAA regulations.
β’ Travel within the region and/or state using a personal vehicle may be necessary.
