This is a fully remote position, open to applicants in United States.

📋 Description

• Deliver consultation and management for patients facing high-risk and complex dual medical and behavioral diagnoses.

• Offer psychosocial counseling and support services to the Sharp Community Medical Group population.

• Promote the integration and continuity of patient care in both medical and behavioral health contexts.

• Guide patients through their behavioral health benefits, available programs, the Sharp HealthCare system, and community resources.

• Mobilize resources for patients and their families to enhance their medical and behavioral healthcare benefits and overall quality of life.

• Conduct psychosocial assessments and identify key clinical concerns.

• Document assessments, consultations, recommendations, interventions, care plans, and patient progress thoroughly.

• Consult and communicate with primary care providers, behavioral health specialists, and members of the multidisciplinary team.

• Create and engage in patient care conferences addressing challenging or high-risk psychosocial cases.

• Refer patients and families to appropriate hospital and community resources.

• Facilitate patient transitions across the entire care continuum.

• Provide consultation and intervention during crisis situations; assist staff in de-escalation efforts.

• Coordinate critical incident debriefings and develop nursing education strategies.

• Advocate for patients and their families while reporting any mandatory protective concerns.

• Develop, implement, evaluate, and modify medical and behavioral health care management plans as necessary.

• Utilize individual, conjoint, and group treatment modalities effectively.


⛳️ Requirements

• Bachelor's Degree in Social Work or a related field in behavioral health.

• Preferred experience in Behavioral Health.

• California BBS Licensed Clinical Social Worker (LCSW) is preferred.

• Theoretical and practical understanding of psychosocial processes.

• Strong skills in assessment and clinical counseling.

• Exceptional oral and written communication abilities.

• Excellent problem-solving capabilities.

• Proficient documentation skills.

• Strong collaboration and negotiation skills.

• Knowledge of HIPAA regulations, patient privacy, confidentiality, and compliance requirements.

• Ability to perform crisis intervention and apply problem-solving techniques.

• Awareness and sensitivity to cultural and ethnic diversity.
