Medical Social Worker

This is a fully remote position, open to applicants in California.

📋 Description

• Oversee the daily operations of the social services team, including the implementation of policies, procedures, and coordination of care with other clinical providers.

• Recruit, onboard, and mentor social workers while providing clinical supervision, evaluating practice, and fostering professional development.

• Supervise the assessment, planning, and evaluation processes for social services care.

• Ensure that clinical documentation complies with state, federal, and ACHC standards.

• Collaborate with the Executive Director on the annual budget and resource planning for the department.

• Develop expertise in community resources and facilitate connections for patients and referral sources to appropriate support services.

• Organize in-service training and continuing education opportunities.

• Participate in quality improvement initiatives and seek out methods to enhance patient care.

• Carry out additional duties as necessary to support patient services.

⛳️ Requirements

• Master's degree in Social Work (MSW) from an accredited institution.

• Current California license as a Licensed Clinical Social Worker (LCSW).

• Minimum of 3 years of management experience, ideally in home care settings.

• Comprehensive understanding of healthcare regulations, ACHC standards, and business operations.

• Exceptional communication skills with the ability to establish trust with patients, families, staff, and the community.

• Self-motivated, organized, and adaptable, demonstrating initiative and the ability to collaborate effectively with others.
