Manager, Digital Population Health Applications Transformation

This is a fully remote position, open to applicants in New Hampshire, +4 more states.

📋 Description

• Oversee the planning, execution, and integration of enhancements, optimization initiatives, and transformational projects across Epic digital population health applications.

• Enhance the digital front door and patient engagement strategy through cohesive digital experiences that encompass patient access, communication, engagement, and comprehensive health management.

• Drive the advancement of population health applications that support proactive care management, quality performance, and outcomes based on value-based care.

• Propel improvements through project execution, workflow redesign, platform optimization, automation, and modernization efforts.

• Collaborate with leaders in Population Health, Clinical, Access, operations, and IT to transform priorities into actionable roadmaps, funded projects, and measurable results.

• Provide direct leadership and accountability for designated teams, services, or functional areas.

• Establish priorities and oversee daily operations.

• Manage, assign tasks, assess performance, give feedback, and promote staff development.

• Address operational challenges, incidents, and service interruptions, escalating issues as needed.

• Ensure that assigned services adhere to organizational policies, regulatory standards, and governance requirements.

⛳️ Requirements

• A Bachelor’s degree in Healthcare, Information Technology, Business, or a related discipline is required, or substantial equivalent experience, certifications, and education.

• ITIL Foundation Certification must be obtained within six months of hiring.

• Active Epic certification in at least one of the following: MyChart, Healthy Planet, Cheers, HIM.

• A minimum of five years of combined experience in IT and Healthcare.

• At least three years of experience in managing IT Infrastructure.
