This is a fully remote position, open to applicants in Texas.
• Develop, evaluate, and manage comprehensive care activities for individuals with substantial mental and behavioral health challenges.
• Assess the service needs of members and create or contribute to care and service plans.
• Provide education to members, families, and caregivers regarding available services, benefits, preventive care, treatment options, referrals, medications, and healthcare instructions.
• Assess complex and high-risk members and propose appropriate care plans.
• Serve as a liaison and advocate for members between families, healthcare providers, and facilities or agencies.
• Assist members facing issues such as depression, bipolar disorder, schizophrenia, borderline personality disorder, PTSD, substance use disorder, self-injurious behavior, and psychiatric inpatient care.
• Conduct regular home and other site visits to evaluate needs and collaborate with available resources.
• Offer specialized knowledge and operational support for behavioral health initiatives, crisis interventions, mental health first aid, field safety, de-escalation techniques, and medication oversight.
• Educate members about and coordinate access to medical, behavioral, and social community resources.
• Manage service authorizations for various services, including meals, employment, housing, foster care, transportation, and daily living activities.
• Facilitate referrals and assist in identifying accessible providers, specialists, and community resources.
• Ensure accurate documentation within the electronic care management system and uphold compliance with clinical and regulatory standards.
• Provide leadership feedback aimed at enhancing quality of care and service delivery.
• Carry out additional duties as assigned while adhering to relevant policies and standards.
• A Master's degree in Mental Health or Social Work, or graduation from a recognized nursing program.
• 2–4 years of relevant experience.
• Must be a Licensed Behavioral Health Professional or RN as per state contract stipulations, including LCSW, LMFT, LMHC, LPC, or RN with behavioral health experience.
• For the Sunflower plan: must hold LMSW, LCSW, LMFT, LMHC, LPC, or RN with behavioral health experience.
• Regular home and/or site visits required, at least once a month.
• For the Arkansas Total Care plan: must have a driver's license, complete a child and adult maltreatment check prior to hiring and on a recurring basis, pass a drug screening at hiring and periodically, reside in Arkansas or a border city, and be willing to travel 5% of the time.
