
LTSS Service Care Manager β Behavioral Health

This is a fully remote position, open to applicants in Texas.
β’ Design, evaluate, and manage comprehensive care management initiatives for individuals with significant mental and behavioral health challenges.
β’ Assess member service requirements and create or contribute to care and service plans.
β’ Provide education to members, families, and caregivers regarding available services, benefits, preventive care, treatment options, referrals, medications, and instructions from healthcare providers.
β’ Assess complex and high-risk members and propose appropriate care plans.
β’ Act as a liaison and advocate for members between families, physicians, and various facilities/agencies.
β’ Assist members dealing with mental and behavioral health issues, including depression, bipolar disorder, schizophrenia, borderline personality disorder, PTSD, substance use disorder, self-injurious behavior, and psychiatric inpatient admissions.
β’ Conduct regular home and site visits, at least once a month, to evaluate needs and engage with resources.
β’ Offer subject matter expertise and operational support for behavioral health initiatives, including crisis intervention calls, mental health first aid training, field safety, de-escalation techniques, and medication monitoring.
β’ Educate on and coordinate access to medical, behavioral, and social community resources.
β’ Manage service authorizations for offerings such as meals, employment, housing, foster care, transportation, and daily living activities.
β’ Facilitate referrals and identify relevant providers, specialists, and community resources.
β’ Ensure precise documentation in the electronic care management system.
β’ Uphold adherence to clinical, state, and federal regulations.
β’ Provide leadership insights to enhance the quality of care and cost-effective service delivery.
β’ Execute other assigned responsibilities.
β’ Master's degree in Mental Health or Social Work, or graduation from an accredited nursing program.
β’ 2β4 years of relevant experience.
β’ Licensed Behavioral Health Professional or RN in accordance with state contract requirements, such as LCSW, LMFT, LMHC, LPC, or RN with experience in behavioral health.
β’ For the Sunflower plan: Licensed Behavioral Health Professional or RN meeting state contract standards, including LMSW, LCSW, LMFT, LMHC, LPC, or RN with behavioral health experience.
β’ For the Arkansas Total Care plan: must possess a driver's license, undergo child and adult maltreatment checks prior to hiring and periodically thereafter, and complete drug screenings at hiring and regularly thereafter.
β’ For the Arkansas Total Care plan: must reside in Arkansas or a neighboring city.
β’ Willingness to travel 5% for the Arkansas Total Care plan.
