LTSS Service Care Manager – Behavioral Health

This is a fully remote position, open to applicants in Kansas.

📋 Description

• Develops, evaluates, and coordinates comprehensive care management activities, primarily focusing on populations with significant mental and behavioral health needs, to achieve quality and cost-effective healthcare outcomes.

• Assesses member service requirements and contributes to the creation of care plans or service plans, while educating members, their families, and caregivers about the available services and benefits to address member needs.

• Analyzes the needs of complex and high-risk members with mental or behavioral health challenges and recommends a tailored plan of care for optimal outcomes.

• Serves as a liaison and advocate for members, facilitating communication between the member/family, physician, and facilities/agencies.

• Supports members primarily dealing with mental and behavioral health issues, including those experiencing (or with a history of) major depression, bipolar disorders, schizophrenia, borderline personality disorder, post-traumatic stress disorder, substance use disorder, self-injurious behavior, and psychiatric inpatient admissions.

• Conducts regular home visits and/or other site visits (at least once a month) to assess member needs and collaborate with necessary resources.

• Offers and/or facilitates education for long-term care members and their families or caregivers on subjects such as preventive care, procedures, healthcare provider instructions, treatment options, referrals, prescribed medication regimens, and healthcare benefits.

• Provides expertise and operational support for mental and behavioral health-focused activities, including managing crisis calls, mental health first aid training, field safety, de-escalation practices, and monitoring psychotropic and other medications.

• Educates and connects members with community resources that encompass medical, behavioral, and social services.

• Coordinates service authorization for members and care managers based on assessments and care plans for various services (e.g., meals, employment, housing, foster care, transportation, activities of daily living).

• Ensures appropriate referrals are made based on individual member needs and assists in identifying providers, specialists, and community resources.

• Guarantees that identified services are accessible to members.

• Maintains precise documentation and upholds the integrity of care management activities in the electronic care management system.

• Works to ensure adherence to clinical guidelines and current state and federal regulations.

• Provides feedback to leadership regarding opportunities to enhance the quality of care and service delivery for long-term care members in a cost-effective manner.

• Performs additional duties as assigned.

• Adheres to all policies and standards.

⛳️ Requirements

• A Master's degree in Mental Health or Social Work, or graduation from an Accredited School of Nursing, along with 2 to 4 years of relevant experience is required.

• Licensure as a Behavioral Health Professional or RN is mandated based on state contract requirements, such as LMSW, LCSW, LMFT, LMHC, LPC, and RN with Behavioral Health experience.
