LTSS Service Care Manager

This is a fully remote position, open to applicants in Pennsylvania.

📋 Description

• Develops, evaluates, and coordinates comprehensive care management activities for long-term care members.

• Creates or assists in the formulation of individualized service care plans/service plans.

• Assesses member needs and available resources, recommending or facilitating suitable care plans.

• Identifies providers, specialists, and community resources for long-term care support.

• Facilitates coordination among members, families, caregivers, and care provider teams.

• Offers resource assistance for employment, housing, participant direction, independent living, justice, and foster care requirements.

• Monitors care plans, member status, and outcomes, suggesting updates based on the evolving needs of members.

• Engages with long-term care healthcare providers and collaborative partners.

• Collects, documents, and maintains member information and care management activities to ensure regulatory compliance.

• Conducts home and other site visits to evaluate member needs and collaborate effectively with healthcare providers and partners.

• Educates members and their families/caregivers regarding procedures, provider instructions, service options, referrals, and healthcare benefits.

• Provides leadership insights on opportunities to enhance the quality of care and promote cost-effective service delivery.

• Performs additional duties as required.

⛳️ Requirements

• Bachelor's degree and 2–4 years of relevant experience, or equivalent expertise gained through applicable knowledge, duties, scope, and skills.

• Candidates must reside in Pennsylvania.

• Prior assessment experience is highly preferred.

• Experience with Community Health Choices (CHC) is highly preferred.

• Willingness to travel approximately 50% to nursing facilities.

• Capability to perform home and/or other site visits.
