This is a fully remote position, open to applicants in Kansas.
• Create, evaluate, and manage comprehensive care management activities for long-term care members.
• Design or assist in the development of tailored service care plans/service plans.
• Provide education to members and their families/caregivers regarding available services and benefits.
• Assess member needs and available resources; recommend or facilitate suitable plans.
• Identify providers, specialists, and community resources for long-term care support.
• Facilitate coordination among members, families/caregivers, and care provider teams.
• Offer resource support for employment, housing, participant direction, independent living, justice, and foster care requirements.
• Oversee care plans, monitor member status and outcomes; suggest updates based on evolving needs.
• Engage with long-term care healthcare providers and partners.
• Gather, document, and preserve member information and care management activities to ensure regulatory compliance.
• Conduct home visits and other site visits as necessary.
• Promote education on procedures, provider instructions, service options, referrals, and healthcare benefits.
• Provide leadership feedback on the quality of care and cost-effective service provision.
• Carry out additional duties as assigned.
• Bachelor's degree with 2–4 years of relevant experience.
• Alternatively, a Bachelor's degree in health, human, social work, or education services with one or more years of qualifying experience.
• Alternatively, a high school diploma or equivalent alongside three years of qualifying experience in case management for the elderly, including management of behavioral health conditions, individuals with physical or developmental disabilities, or the HIV/AIDS population.
• Equivalent experience gained through relevant knowledge, responsibilities, and skills reflective of the position's level may be considered.
• Must live within 30 miles of South Johnson, Miami, and/or Franklin County, Kansas.
• Proficient in computer use.
• Exceptional communication and customer service abilities.
• Familiarity with HCBS waivers and behavioral health.
• Strong organizational and time management skills.
• Ability to collaborate effectively as part of a team.
• Knowledge of community-based referrals and resources.
• Excellent problem-solving and assessment capabilities.
• Capacity to work independently in a community-oriented setting.
• Flexibility to adapt to client and program needs.
• Availability to work Monday through Friday, from 8:00 AM to 5:00 PM, with the possibility of evening and weekend shifts as needed.
