LPN/LVN Care Coordinator

This is a fully remote position, open to applicants in United States.

📋 Description

• Provide education to patients over the phone regarding palliative care.

• Arrange appointments for patients seeking palliative services.

• Engage directly with patients, healthcare providers, and their families.

• Manage clinical files to finalize palliative visits and input notes into systems.

• Allocate part of the day to phone interactions and part to administrative tasks.

• Assist patients in comprehending the benefits of palliative care.

• Link patients who require the program with palliative care providers.

• Address patient inquiries about palliative care and the associated program.

• Ensure documentation of patient encounters and proper triage of patient needs.

• Conduct follow-ups with patients to guarantee they receive exceptional care from Empassion's provider network.

• Review and finalize completed cases on a daily basis.

• Balance responsibilities between clinical triage and patient communications.

⛳️ Requirements

• At least 3+ years of experience as a Licensed Practical Nurse (LPN).

• A minimum of 3+ years in reading and updating clinical documentation.

• At least 1+ years of relevant experience in a customer service call center.

• Ability to receive calls from patients, caregivers, and healthcare providers.

• Excellent communication and writing abilities.

• Strong attention to detail.

• Comfortable using technology and thriving in a dynamic startup environment.

• Reliable Wi-Fi connectivity is essential.

• Proficiency in Spanish is a plus, but not mandatory.
