This is a fully remote position, open to applicants in Connecticut, +8 more states.

📋 Description

• Assess and provide treatment to patients under the guidance of a physician.

• Perform outreach to members who have recently been discharged from the ER for follow-up and referral to CareBridge services.

• Enroll members into CareBridge services and arrange visits with nurse practitioners.

• Triage patients, carry out nursing assessments, and refer to an acute nurse practitioner.

• Review medical records, interview patients, and take vital signs.

• Clarify procedures, execute treatments, and administer prescribed medications.

• Assist in facilitating patient transfers to different levels of care.

⛳️ Requirements

• An LPN or LVN is required, along with a minimum of 2 years of experience as an LPN/LVN; or any combination of education and experience that would provide an equivalent background.

• A current LPN/LVN license in the relevant state is mandatory.

• Successful completion of a Tuberculosis test is required for this role.

• Bilingual (English/Spanish) is preferred.

• Prior experience in behavioral health, care coordination, community health, transitional care, and/or chronic disease management is highly preferred.

• Must live within a reasonable commuting distance from one of the job locations unless accommodations are granted as required by law.

• Ability to work four 10-hour shifts weekly, with every other weekend required.

• Availability to work from 8 am to 7 pm CST.
