This is a fully remote position, open to applicants in Pennsylvania.
• Identify, facilitate, and secure access to healthcare, social service benefits, and community resources.
• Assist members in navigating efficient and effective care pathways.
• Coordinate appointments and arrange transportation for both physicians and non-physician providers.
• Handle incoming calls, make outgoing calls, and respond to voicemail inquiries.
• Assess and identify the medical, behavioral, social, emotional, and financial needs of members.
• Connect high-risk members with suitable community resources and provide health education.
• Develop relationships with members, families, and caregivers while offering support towards healthcare objectives.
• Provide emotional assistance and refer members for more intensive psychosocial interventions when necessary.
• Complete educational sessions, interventions, surveys, and assessments for members.
• Offer condition-specific education, medication adherence support, preventive care advice, and assistance with health benefits navigation.
• Address care gaps, reinforce provider care plans, and advocate for evidence-based practices.
• Collaborate with interdisciplinary teams to enhance quality outcomes and improve member experiences.
• Identify and report any quality-of-care concerns.
• Ensure member confidentiality is maintained.
• Document activities of care navigation in the care management documentation system.
• Attend company and departmental meetings, as well as training sessions.
• Retrieve and assign referrals and complete monthly reconciliation reports.
• Minimum of three (3) years of recent and relevant experience in health and wellness promotion, inpatient, or another appropriate clinical setting.
• Preferred experience in Behavioral Health.
• Patient Navigation certification is preferred or must be obtained within one year of employment.
• Active Licensed Practical Nurse license or degree in a healthcare-related field with three years of directly related experience.
• Strong critical thinking and problem-solving abilities.
• Excellent skills in written and oral communication, active listening, and organization.
• Proficiency in Microsoft Office Suite.
• Ability to utilize a computer system while engaging in telephonic conversations.
• Strong customer service skills, demonstrating tact and diplomacy.
• Capable of prioritizing tasks and completing responsibilities accurately and in a timely manner.
• Ability to work independently and collaboratively within an interdisciplinary team.
• Knowledge of medical, behavioral, and social systems.
• Familiarity with NCQA standards, DMAA standards, CMSA Standards of Practice, Act 68, and CMS regulations.
• Understanding of medical treatment modalities and best practice guidelines.
• Knowledge of adult learning principles, motivational interviewing, and intrinsic coaching techniques.
