LPN

This is a fully remote position, open to applicants in Virginia.

📋 Description

• Oversee RPM dashboards and promptly respond to alerts (such as blood pressure, glucose, weight, etc.)

• Regularly contact patients to evaluate symptoms and encourage device usage

• Facilitate follow-ups with nurse practitioners, primary care providers, and caregivers

• Record patient interactions in EMR and internal systems

• Resolve device issues to maintain patient compliance

• Act as the main point of contact for a designated group of patients

• Assist with patient enrollment and onboarding as required

• Collaborate daily with nurse practitioners, intake team, and support personnel

⛳️ Requirements

• Current Virginia LPN license is preferred, or the ability to obtain it swiftly

• Strong clinical judgment along with clear and professional phone communication skills

• Highly organized and adept at navigating multiple platforms

• Preferably experienced in RPM, telehealth, or care coordination

• Genuine interest in preventive care and fostering patient engagement

• Capability to thrive in a fast-paced, structured environment
