Licensed Registered Nurse Care Coach

This is a fully remote position, open to applicants in Illinois.

📋 Description

• Conduct monthly calls with a full caseload of Medicare patients suffering from two or more chronic conditions by utilizing specialized care management software.

• Establish and sustain rapport with patients through SMART goals and education on self-management.

• Enhance and implement patient Plans of Care by updating medications, appointments, biometrics, symptoms, and interventions.

• Link patients to community resources, which may include transportation, personal care, prescription/DME assistance, and social services.

• Perform Transitional Care Management activities for high-risk patients released from hospitals and emergency rooms.

• Address preventive care gaps by promoting wellness visits, vaccinations, cancer screenings, and follow-up or specialist appointments.

• Ensure documentation is accurate, timely, and complete.

• Prioritize and finalize case tasks punctually while adhering to quality, compliance, and performance standards.

⛳️ Requirements

• Proficient in English.

• A current and unrestricted Illinois RN license is mandatory.

• At least 3 years of experience as a Registered Nurse.

• Proficient in electronic health records and web-based applications.

• Self-motivated and capable of working independently with minimal supervision while achieving performance metrics.

• Strong passion for nursing and enhancing patient outcomes.

• Comfortable with technology and enthusiastic about learning and utilizing new software.

• Excellent organizational and time management abilities.

• Strong communication and telephonic skills.

• Solid critical thinking and problem-solving capabilities.

• Must possess a strong internet-connected computer; equipment is not supplied by the company.

• Availability of at least 20 hours during daytime between 9am and 6pm Eastern, Monday-Sunday.

• Ability to manage one's own schedule using the company's software.

• Responsible for personal equipment, taxes, and insurance as a 1099 contractor.
