This is a fully remote position, open to applicants in Virginia.
• Deliver consultation-focused behavioral health services to patients in partnership with primary care providers.
• Function in a consultative capacity within an integrated behavioral health primary care framework.
• Offer behavioral intervention recommendations to primary care providers making referrals.
• Execute targeted interventions through same-day virtual warm handoffs and scheduled video consultations.
• Conduct focused assessments and engage in collaborative care and treatment within the primary care setting.
• Perform risk assessments in crisis scenarios.
• Provide evidence-based interventions addressing mental health symptoms, behavioral health challenges, chronic disease management, and general health promotion.
• Implement collaborative, stepped, and measurement-based care with appropriate follow-up.
• Collaborate with primary care team members to develop clinical pathways and psychoeducational classes.
• Create patient education materials for prevalent behavioral health conditions.
• Offer collaborative decision support and interdisciplinary consultation.
• Educate primary care team members on brief behavioral and cognitive behavioral interventions.
• Support medication adherence and its effectiveness.
• Assist with triage, care coordination, and access to external resources.
• Engage in clinical training, quality improvement initiatives, and staff meetings.
• Maintain clinical records and necessary paperwork.
• Ensure virtual visibility and availability to the primary care team during clinic hours.
• Provide curbside consultation via chat or phone.
• Conduct all patient care through HIPAA-compliant video telehealth.
• Must reside in Virginia and provide all care from within the Commonwealth.
• A Master’s degree in Social Work from a CSWE accredited program or a Master’s degree in Counseling from a CACREP or CORE accredited program is required.
• Licensed to practice as an LCSW or LPC in Virginia, or must secure Virginia licensure within 18 months if actively licensed in another state.
• BLS Certification must be obtained within 90 days of hire.
• Experience in delivering clinical interventions for a wide range of behavioral health conditions.
• Familiarity with medically-related issues.
• Experience with pediatric populations and families in pediatric practices.
• Interest in working in a dynamic primary care environment and within an interdisciplinary team.
• Proficiency in using personal computers and computerized scheduling systems.
• Reliable high-speed internet is required.
• A private, confidential workspace suitable for clinical telehealth is necessary.
• Flexibility to provide weekend and/or evening coverage as part of a seven-day clinic rotation.
• Strong interpersonal skills with patients and the primary care team.
• Willingness to maintain adaptability in the clinic schedule.
• Ability to adopt a learning and growth mindset towards new topics and challenges.
• Passion for working in primary care and as a team member.
• Computer equipment will be supplied.
• A base salary complemented by a wRVU-based incentive model for the first two years of employment.
