Licensed Practical Nurse, Virtual

This is a fully remote position, open to applicants in New Jersey.

📋 Description

• Conduct telephonic and virtual clinical intakes, health screenings, and wellness checks with patients and caregivers.

• Execute telephonic triage for symptom-based inquiries using established protocols and algorithms, guiding patients to appropriate care levels.

• Act as a primary virtual contact for patients, families, and interdisciplinary care teams.

• Review and modify individual care plans under the guidance of Registered Nurses (RNs) or physicians.

• Monitor remote patient data, vital sign trends, and clinical records to identify service gaps or status changes.

• Facilitate prescription refills, orders, and clinical authorizations in collaboration with physicians, pharmacies, and insurance providers.

• Ensure accurate and timely clinical documentation within Electronic Health Record (EHR) systems.

• Engage in virtual case conferences, team huddles, and quality/accreditation reviews.

⛳️ Requirements

• An active, unencumbered LPN license in your residing state is mandatory.

• Compact Licensure is preferred.

• A minimum of 1–2 years of clinical experience is required.

• Experience in telehealth, telephonic triage, or remote care coordination is preferred.

• Strong clinical assessment, telephone triage, and critical-thinking abilities are essential.

• Excellent verbal, written, and empathetic communication skills for telephone and video interactions.

• Proficiency in Google Workspace, EHR systems, and secure web-based communication tools.

• A dedicated home office setup with reliable, high-speed internet connection is necessary.

• High attention to HIPAA privacy standards is required.
