Licensed Practical Nurse

This is a fully remote position, open to applicants in California.

📋 Description

• Deliver direct patient care in alignment with the state's Nurse Practice Act.

• Execute and adhere to personalized care plans set forth by the RN.

• Provide education to patients and their families regarding self-care strategies and diagnosis-related information.

• Ensure accurate and timely documentation of care provided.

• Work collaboratively with the care team, including OT, PT, HHA, and other disciplines, while effectively communicating patient needs to the RN or Clinical Supervisor.

• Safely utilize supplies and equipment, following aseptic techniques.

• Carry out additional responsibilities as necessary for patient services.

⛳️ Requirements

• Graduate from an accredited practical or vocational nursing program.

• Hold an active LPN or LVN license in California.

• Possess 3+ years of nursing experience, preferably with a background in home health or med/surg.

• Maintain current CPR certification from AHA.

• Have a valid driver’s license, a reliable insured vehicle, and the ability to travel to patient residences.

• Exhibit strong communication skills along with a collaborative, patient-first approach.
