This is a fully remote position, open to applicants in Arizona, +10 more states.
• Support the ECM Sales/Marketing and Clinical Outreach Specialists teams in organizing and enrolling patients in various programs.
• Enhance the patient enrollment process by fostering positive relationships with teams, patients, and their families.
• Perform direct outreach to potential clients to inform them about available services and facilitate their enrollment.
• Assist with the intake and admission process for incoming patients, providing outreach support when necessary.
• Deliver accurate and continuous assessments of patient status during the admission phase.
• Maintain databases, logs, census sheets, resource/contact lists, schedules, and departmental information sheets.
• Conduct insurance eligibility checks and assign clients to the Clinical Outreach Specialist.
• Collaborate cross-functionally with sales, outreach, care delivery, and RCM teams to streamline the enrollment workflow.
• Document all interactions related to the enrollment process in the operating system.
• Attend staff, census, and other meetings, including weekly Outreach Team meetings.
• Achieve or surpass established departmental KPIs.
• Handle incoming calls and emails.
• Undertake additional duties to support organizational needs.
• A minimum of 2 years of prior intake or admission experience in a high call volume setting.
• Active CMA/CCMA certification issued by an accredited organization is required upon application.
• Previous experience working remotely is preferred.
• A quiet and secure workspace to ensure member privacy is essential.
• Over 2 years of experience in palliative care, home health, hospice, and/or hospital discharge planning or case management is highly preferred.
• More than 2 years of experience working with or directly with one or more large safety net hospitals in the region is preferred.
• Must be available to work Monday through Friday from 8:30 am to 5:00 pm Pacific Time, with potential evening and weekend shifts.
• Strong understanding of cultural competency concerning the target population.
• Excellent verbal and written communication abilities.
• Proficient computer skills and a working knowledge of the internet.
• Capability to independently learn and utilize Google Suite and Salesforce.
• Background in training as or alongside social workers, nurses, medical assistants, chaplains, medical scribes, or other educational or work experiences that demonstrate fluency in clinical language and settings.
• Skills in project development and the ability to work autonomously.
• Bilingual proficiency in English and Spanish is highly preferred.
• Must pass background screening, which may include a drug test or other health assessments.
• Employees in patient-facing roles must be vaccinated against influenza; accommodations for religious or medical reasons may be considered.
• Must comply with identity and work eligibility verification using E-Verify upon hire.
• Must be legally authorized to work in the United States and indicate if employment visa sponsorship is needed.
