Intake Coordinator

This is a fully remote position, open to applicants in United States.

📋 Description

• Accurately and promptly process home health referrals from agencies.

• Input data and client details derived from referrals.

• Perform basic diagnosis coding based on the information from referrals.

• Utilize the Welcome Call Script to coordinate care for homecare patients.

• Identify referrals from the Clinical Intake team.

• Engage with agency referral sources.

• Assess patients’ clinical and service requirements.

• Work collaboratively with the Intake team to ensure optimal patient care and timely delivery of clinical services.

⛳️ Requirements

• Required experience with Homecare Homebase (HCHB).

• Minimum of one year in a customer service role.

• An Associate’s Degree is preferred.

• At least one year of experience in Home Health is preferred.

• Successful completion of a pre-employment drug screening is necessary.

• Must be legally authorized to work in the United States.
