Innovations Waiver Care Manager

This is a fully remote position, open to applicants in North Carolina.

📋 Description

• Deliver comprehensive care coordination and oversight for individuals with primary intellectual or developmental disabilities, as well as potential secondary mental health, substance use, or intricate medical needs.

• Carry out assessments, monitoring, and intricate care coordination while ensuring compliance with waiver requirements.

• Support integrated care management through collaboration across multiple disciplines and referrals.

• Employ person-centered planning techniques to collect information and meet the unique needs of each member.

• Provide education and support to members regarding their rights and obligations.

• Complete Discovery activities, which include information gathering and assessments.

• Inform members and their legally responsible parties about Medicaid, state-funded services, natural supports, community resources, and available service options.

• Assist members and their legally responsible parties in making objective choices regarding service providers.

⛳️ Requirements

• Must fulfill QP status.

• Fully licensed by the North Carolina governing board overseeing Human Services professions; OR a fully licensed RN authorized to practice in North Carolina by the NC Board of Nursing with four years of full-time experience in MH/DD/SA; OR a master's degree in a human service field with one year of full-time MH/DD/SA experience; OR a bachelor's degree in a human service field with two years of full-time MH/DD/SA experience; OR a bachelor's degree in another discipline with four years of full-time MH/DD/SA experience.

• Substance abuse professionals must comply with the specified supervised experience requirements for alcoholism and drug abuse counseling.

• For members requiring LTSS, a minimum of two years of prior experience in LTSS and/or HCBS coordination, care delivery monitoring, and care management is necessary.

• Must possess a valid driver's license.

• Must reside within Trillium's North Central Region: Beaufort, Dare, Edgecombe, Greene, Hyde, Nash, Pamlico, Pitt, Tyrrell, Washington, or Wilson counties.

• Must be capable of traveling within the designated area as needed.

• Must successfully pass a drug test as a condition of employment.
