This is a fully remote position, open to applicants in Washington.

📋 Description

• Conduct In-Home Health Assessments in the residences of members.

• Travel predominantly, up to 100% of the time, within a specified coverage area.

• Carry out approximately 45-minute individual visits.

• Evaluate medications and review medical histories.

• Execute physical assessments and perform ordered diagnostic tests, including diabetic eye examinations and spirometry.

• Utilize an iPad and follow clinical workflows for each assessment.

• Facilitate virtual visits as necessary.

• Deliver Signify Health Diagnostic & Preventive Services products.

• Establish trustworthy relationships to enhance health outcomes.

• Make referrals to health plans and encourage the involvement of family members when suitable.

• Ensure that primary care physicians are informed about members’ health status and quality of life.

• Travel both locally and within licensed states, which may include potential overnight hotel stays, car rentals, and flights.

⛳️ Requirements

• Possess an active, unrestricted license(s) in the applicable coverage area(s).

• Capable of obtaining and maintaining multi-state licenses in the coverage area as necessary.

• Must be able to comfortably and safely perform tasks in homes that may include household pets, secondhand smoke, and other potential airborne allergens or irritants.

• Flexibility to travel locally and within licensed states.

• Ability to carry out in-home health assessments, including reviewing medications and medical histories, conducting physical evaluations, and performing diagnostic tests as required.
