In-Home Traveler Nurse Practitioner, Physician Assistant

This is a fully remote position, open to applicants in Virginia.

📋 Description

• Conduct Health Evaluations in the homes of members.

• Connect with individuals to assist in bridging care gaps.

• Assess medications and review medical histories.

• Perform physical assessments.

• Execute additional diagnostic tests as prescribed by the health plan, such as diabetic eye exams, spirometry, and bone density screenings.

• Utilize an iPad and follow clinical workflows for each evaluation.

• Establish trusted relationships to enhance health outcomes.

• Travel nationwide to connect individuals with the appropriate care.

• Engage in local travel and within licensed states.

• Conduct virtual consultations as necessary.

• Deliver Signify Health services, including Diagnostic & Preventive Services products.

• Primarily work in the private residences of health plan members.

⛳️ Requirements

• Possess an active, unrestricted license(s) in the relevant coverage area(s).

• Board certification is mandatory.

• Capable of obtaining and maintaining multi-state licenses in the coverage area, as required.

• Must be able to cover all counties within the state and be flexible to other business needs.

• Qualifications as a Nurse Practitioner or Physician Assistant are necessary.

• Willingness to travel locally and within licensed states.

• Ability to travel up to 100% of the time within a designated coverage area.

• Prepared to stay overnight in hotels and utilize car rentals and/or flights as necessary.

• Competence to safely perform duties in homes with household pets, secondhand smoke, and potential airborne allergens or irritants.

• Availability for a full-time commitment of 40 hours per week.
