In-Home Traveler Nurse Practitioner, Physician Assistant

This is a fully remote position, open to applicants in United States.

📋 Description

• Conduct In-Home Health Evaluations within members' residences

• Build trusted relationships and engage with members effectively

• Assess medications and review medical histories

• Execute physical evaluations

• Offer additional diagnostic and preventive services, such as diabetic eye exams, spirometry, and bone density screenings

• Utilize an iPad and follow clinical workflows for each evaluation

• Travel locally and within licensed states, which may include potential overnight stays, car rentals, and flights

• Conduct virtual visits as required

• Visit individuals nationwide to connect them with suitable care

• Provide health education and address members' inquiries

• Collaborate as a clinician colleague for Signify Health and meet specified role requirements

⛳️ Requirements

• Active and unrestricted license(s) in the relevant coverage areas

• Board certification (mandatory)

• Capability to obtain and maintain multi-state licenses in the coverage area as needed

• Flexibility to cover all counties within the state and adapt to other business needs

• Qualification as a Nurse Practitioner or Physician Assistant

• Willingness to travel up to 100% of the time within a defined coverage area

• Competence to perform duties in homes that may have household pets, secondhand smoke, and other potential airborne allergens or irritants

• Proficiency in using an iPad and clinical workflows

• Ability to conduct medication and medical history reviews, physical evaluations, and diagnostic tests
